Diabetic Clinic

Toukley Family Practice (TFP) runs a diabetic clinic as part of the comprehensive healthcare services it provides to the community.

This clinic primarily supports the GP as he/she manages the patients’ diagnosis, treatment and the ongoing management. The GP identifies the need for the diabetic clinic and asks the patient to book in at a time convenient to them.

Diabetes is a chronic condition which, if not managed well, can lead to disability and sometimes an early death. Diabetes management requires the skills of several other professionals such as specialist physicians, ophthalmologist or optometrist, diabetes educator/nurse, podiatrist, dietitian, mental health worker, exercise professional and dentist.

In our clinic the patient is the central member of this team care arrangement.

For an individual to be able to be responsible for their diabetes they first must be given the chance to understand the condition, it’s effects on their health and how they can best manage it. With a comprehensive plan of management involving the team mentioned above patients with diabetes can learn to better manage their illness thus ensuring improvement in duration and quality of their lives.

Our diabetic practice nurse is Lesley Hutchinson.

GP REFERRAL REQUIRED.
Initial Assessment

1 hour visit with diabetic nurse/10-15min GP consultation. A full assessment includes:

  • full patient history;
  • medication (where needed;
  • risk factors for complications;
  • lifestyle issues;
  • glycaemic control;
  • physical examination (height, weight, BMI, BP, pulse, U/A, eyes, feet);
  • pathology review;
  • need for GP management plan and team care arrangement and initiation/booking of same.
  • Education is also given at this time along with an opportunity to join Diabetes Australia.
      
PREVENTATIVE MEASURES

        3 - 6 Monthly Visits

30 minute visit with diabetic nurse / 10—15 min GP consultation.

Regular follow-up visits provide a great opportunity to get to know the patient better and understand their fears and concerns about diabetes and include:
  • A review of nutrition, smoking, physical activity and alcohol consumption is undertaken;
  • Examination of blood pressure and pulse;
  • Check BMI, feet, glycaemic control and pathology;
  • Reinforcement and further education depending on patient need is also given.
